Immediate relatives of the above members
Spouse - not more than 50 years old.
Children - 18-21 years old & single
Marriage contract and medical certificate - Spouse
Birth Certificate and Medical Certification - Dependent children
Fill up and submit application form
Pay membership fees and subscription dues
